By Internet: To pay by credit card using our secure online payment portal,

By Phone: To pay by credit card over the phone, please contact our Customer Service Department Monday through Friday, between the hours of 8:00 a.m. and 4:00 p.m.
By Mail: We accept payments by check, money order, or major credit card. To ensure proper processing, please include the patient’s full name and account number on all payments. For credit card transactions, please include:
- Your credit card number
- Your expiration date
- Your signature
